The U.S. Chemical Safety and Hazard Investigation Board (CSB) released its final report into the fatal explosion and fire at the KMCO production facility in Crosby, Texas, USA. On April 2, 2019, KMCO was producing sulfurized isobutylene as a lubrication additive.
